Work activities information sheet

If you wish to contact an employee’s GP about their ability to continue carrying out manual handling tasks, you can attach a sheet that gives detailed information on their work activities. Ours can get you started.

A return to work

Much has been made of the rehabilitation of long-term sick employees back into the workplace, but it’s not always obvious where to start. This is particularly the case where the absence is linked to a perceived inability to carry out manual handling tasks. For this reason, we have devised a Work Activities Information Sheet that you can send off to the employee’s GP for them to complete.

Completing the form

Each row on the form outlines what the employee’s job involves in terms of the weight of the items moved, how far they are carried and how frequently etc. The columns can be ticked to signify what type of item is handled, e.g. crate or cage-style trolley. The GP then can fill out their recommendations within each box.
