Unsafe equipment report

On occasion, even the best maintenance regime can fail to stop defects from appearing. Should this happen, why not use our report form to help ensure that they’re reported and rectified quickly?

Reporting defects

Whilst regular maintenance will help ensure that machinery and equipment is maintained in a safe condition and doesn’t develop faults, the unexpected can still happen. For this reason, we have introduced an Unsafe Equipment Report. Its purpose is to enable users to report suspected defects in a timely manner and with enough detail to enable repairs to be carried out with the minimum of downtime.

Using the form

The first part of the form allows the machinery/equipment user to provide details of the machine, as well as providing a brief description of the suspected or known fault. In order to maintain an audit trail, there is also space to provide information as to who the fault was reported to and what remedial action is required. If the repair is to be carried out in-house, there is a separate section that allows the engineer requires that a copy of the engineer’s report is attached.
