Flow chart - failure in lock out tag out process

If you have a LOTO policy, it’s essential that you follow your commitments to control the risk of working with live electrical equipment. However, mistakes can be made or staff might abuse the padlock system so you will need to know how to act in a consistent manner to adhere to your policy.
