Annual subcontractor appraisal questionnaire

If you subcontract, it’s important to check that any businesses you use are appropriately insured and capable of completing the work in a safe manner. Achieve this by using our document.

It isn’t difficult

Using our Annual Subcontractor Appraisal Questionnaire will help you to find out everything you need to know about a business you wish to use. The completed questionnaire will contain the evidence you need to prove your case if you ever need to do so.

Note. As the title of this document suggests, this should be an annual process. This will mean that you have up-to-date contact information for health and safety matters and details of their current insurance cover.

What’s covered?

The first section of the document “Company/personnel details” asks for details of staff and others, e.g. a retained consultant who deals with health and safety issues. It also asks the sub-contractor to provide a summary of any qualifications they hold.

What can you do?

The second section asks the subcontractor to list the services they are able to offer. This will help you to identify which jobs can be passed to them in the future.

References

One of the best ways of finding out whether the contractor is as good as they claim to be is to ask for references. Our document states “Please supply contact names and addresses of two organisations that you have carried out similar work for in the past 18 months.” We would suggest that you follow up at least one of the references provided.

Safety

The last set of questions asks for confirmation of how health and safety risks are managed.
